At Commonwealth Oral and Maxillofacial Surgery, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.

Please be advised we are a dental office and are not in network with any medical insurance companies, nor do we file claims with them.

If you are unsure if we are in network with your insurance company please fill out our new patient registration form with your dental insurance information. Once we receive this form we will verify we are in network with your insurance, or you can contact your insurance company directly. If you have any questions or concerns, please ask our team. Please bring your insurance information to your first office visit so that we can expedite reimbursement. If you are being seen for a pathology consultation, have Humana Dental Insurance, or attempting to schedule in an operating room, we will ask for your medical insurance information to have on file.

We will no longer be in network with Aetna dental insurance as of June 27th, 2024.
