At Commonwealth Oral and Maxillofacial Surgery, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.

Accepted Dental Insurance Plans 

We accept most PPO dental insurance plans to help you get the best value out of your plan.  Some examples would be Delta Dental, MetLife, Humana Dental PPO, Anthem Blue Cross Blue Shield PPO, Cigna, and United Healthcare. If you are unsure if we are in Network with your dental insurance, please fill out our new patient registration form with your dental insurance information. Once we receive this form we will verify we are in network with your insurance, or you can contact your insurance company directly. If you have any questions or concerns, please ask our team. We are not in network with any medical insurance companies

Please bring your insurance information to your first office visit so that we can expedite reimbursement. If you are being seen for a pathology consultation, have Humana Dental Insurance, or attempting to schedule in an operating room, we will ask for your medical insurance information to have on file.

We are out of network with Aetna Dental, United Concordia, Medicaid, and most Medicare plans.

Before your procedure we will send in your treatment plan to your insurance company to obtain a predetermination of benefits. A predetermination is an estimate of benefits that your insurance will provide, this will allow us to provide you with your estimated portion for services. Please note that most dental plans pay at 50%-80%. This percentage is determined by the coverage type through your employer or selected contract. Insurance companies set their own fees using their own contracted fee schedule. This means different insurance companies pay at different rates. Please understand that our staff has no control over the fees your insurance company and/or plan has determined for our services. Patient portion is entirely dependent upon your insurance company and will be due at time the services are rendered.
