Scheduling
Commonwealth Oral and Maxillofacial Surgery is a referral based practice, meaning we require a referral to be seen in our office. The only exclusions are wisdom teeth. To schedule an appointment with us please fill out our new patient registration form , once we receive this we will reach out to get you scheduled.
A complex medical history or treatment plan may require a consultation to be done prior to your procedure. Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. If you are scheduled for a consultation appointment your surgery will not be done the same day. A second appointment to provide treatment on another day will be scheduled at the consultation. We will attempt to get a pretreatment estimate from you dental insurance to know how much is estimated to be covered for your surgery.
We try our best to stay on schedule to minimize your waiting. Dr. Acord provides many types of dental services and various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
IMPORTANT:
All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit
Cancellation Policies:
Deposit of $250.00 due at time of scheduling General Anesthesia Procedures. If your procedure is cancelled within 7 calendar days of your surgery date, the deposit becomes non-refundable.
Consultations/ extraction appointments: Failure to modify or cancel an appointment without 24 hour notice before your scheduled appointment time may incur a $25.00 “no-show/cancellation” fee. Appointment cancellations or modifications with little or no notice and appointment no shows impact our ability to make that time available to other patients in need of care. Appointments can be modified or cancelled by calling our office or responding to confirmation texts.
